Acumen and PFCCpartners invite all PFAnetwork members to join us for the Wave 5 Measure Development Project.

The measures in development relate to outpatient management for these chronic conditions:

• Kidney Transplant Management

• Prostate Cancer

• Rheumatoid Arthritis

Tentatively scheduled for July 2022, October 2022, and March 2023, Acumen and PFCCpartners will be conducting focus groups and interviews.

We would like to see PFAnetwork members join in this effort to ensure the patient family perspective is informing the development of these measures. If you have lived experience as a patient or a caregiver to someone experiencing the conditions above, and that person receives Medicare, please consider participating. There will be a stipend provided to participants..
