Acumen, in partnership with PFCCpartners and CMS opened a Field Testing Survey to better understand how episode based cost measures are working in the clinical environment.

PFAnetwork members who are medicare recipients or caregivers of medicare recipients will be invited to share your experiences with the following conditions:

1. Kidney Transplant Management

2. Prostate Cancer

3. Rheumatoid Arthritis

4. Chronic Kidney Disease

5. End-Stage Renal Disease
