Acumen Field Testing Survey

Dates: February 1-29

Our partners at Acumen are requesting all PFAnetwork members to complete a survey related to your lived experience as a patient, family member, or caretaker with regard totwo episode-based cost measures in development.

These measures are:

1. Movement Disorders: Parkinson’s and Related Conditions, Multiple Sclerosis (MS), and Amyotrophic Lateral Sclerosis (ALS);

2. Non-Pressure Ulcers (e.g., arterial, diabetic, venous, and other non-pressure ulcer types). 

If you have 5-10 minutes to provide your insights related to your experience with these conditions, your experience will inform the measures under development. Please feel free to skip any questions that are not relevant to your experience.
